For decades, Joël Le Scouarnec wore the white coat of a respected surgeon. But behind the sterile walls of operating rooms and hospital corridors, he hid a monstrous secret. Known now as “the devil in a white coat,” Le Scouarnec’s name has become synonymous with one of France’s worst medical abuse scandals—a chilling reminder of the dangers that can lurk behind professional respectability.

A Trusted Surgeon

Joël Le Scouarnec was once a decorated gastrointestinal surgeon, working at hospitals across western France, including in Jonzac, Vannes, and Lorient. Over the course of his 30-year career, he performed hundreds of operations and earned the trust of countless patients. But behind closed doors, he was using his position of power and trust to commit unimaginable crimes.

The Shocking Discovery

Le Scouarnec’s dark world began to unravel in 2017 when a 6-year-old girl accused him of sexual assault. The child’s courageous testimony led police to search the doctor’s home. What they found horrified even the most experienced investigators: more than 300 notebooks filled with explicit details of sexual abuse, drawings, and a “black book” chronicling his assaults.

Police later discovered hundreds of images and videos of child abuse on his computer. As they pieced together evidence, a terrifying picture emerged—Le Scouarnec was not a man who had acted on impulse but a systematic predator who had meticulously documented his abuse of children, patients, and even members of his own family over decades.

The Scale of Abuse

French authorities were forced to re-examine medical records and contact past patients. The scale of his crimes was staggering. Initial investigations identified 349 potential victims, some as young as four, many of whom were sedated or unconscious during medical procedures. His victims were male and female, ranging from children to elderly patients.

Le Scouarnec’s abuse reportedly began in the 1980s, meaning institutions may have unknowingly enabled him for years. Several hospitals have since come under scrutiny, with former colleagues and administrators accused of ignoring warning signs or complaints.

The Trial and Conviction

In December 2020, Le Scouarnec was sentenced to 15 years in prison for the sexual abuse of four minors—only the beginning of what may be a series of trials. The French judicial system has continued investigating hundreds more allegations, which may lead to additional prosecutions.

Though Le Scouarnec admitted to some of the charges, he appeared emotionless throughout the proceedings. Many of his victims described the trial as retraumatizing but necessary—to finally expose the man who had stolen so much from them.

Institutional Failure

The Le Scouarnec case has triggered widespread soul-searching in France. How could such a man abuse so many for so long? Did hospital management overlook red flags? Were complaints dismissed too easily. 

In response, France’s health ministry initiated a broader inquiry into hospital practices, with an emphasis on whistleblower protection and abuse reporting procedures. Some former patients are now suing hospitals for negligence, arguing that institutions had a duty to protect them and failed.

The Legacy

Joël Le Scouarnec’s story is not just one of personal horror—it’s a sobering reflection on systemic failure. It’s about the blind trust we place in authority figures, especially in the medical field. And it’s a call to ensure that no white coat, no matter how spotless, can shield someone from accountability.

For the hundreds of victims, justice is still unfolding. For the French medical community and society at large, the case has become a grim reminder: evil can wear a mask of compassion—and it can thrive in silence.
